Kharghar youth gains normal vision despite inflammation in eye at AEHI

Fatima Khan was an insurance agent in a reputed company in Mumbai who lived in Kharghar. Most of her meetings with client required her to travel almost everyday. And, she hardly used to wear her sunglasses or use umbrella etc. One day while meeting a client, her glasses accidentally dropped on the floor and she couldn’t find it immediately. Dr. Jain evaluated her pre-LASIK work up report, it showed that her retina, lens, optic nerve were all in healthy condition.  She had a mild blephritis along with corneal epitheliopathy.

Corneal punctate epitheliopathy (CPE) is a condition wherein the outermost layer (epithelium) of the cornea shows dot-like staining, swelling of epithelium due to surface inflammation. Patients most commonly experience symptoms like sharp pain, foreign body sensation, blurry vision, light sensitivity etc. The main causative factors for the same can be dry eyes, Meibomian gland disease, blephritis etc.

After two weeks, when Fatima came for follow-up, the health of her eyes was much better. It showed enough moisture with no light sensitivity. Dr. Vandana then advised her to continue the same eye drops and visit after a month. Gradually, in the next few months, she was relieved of the symptoms like intermittent blurry vision, eye pain etc.

Belapur woman gets best LASIK treatment despite thin cornea at AEHI

23-year-old Jyoti Nimbalkar was a management employee in a start-up company who lived in CBD Belapur. After reading and listening to various testimonials and stories about LASIK eye surgery from couple of her friends, and office colleagues, she was convinced to visit Advanced Eye Hospital & Institute (AEHI) in Sanpada, Navi Mumbai. Once she reached AEHI, pre-Lasik evaluation was conducted.

The Schirmer test was also performed, the score of which was 9 mm after 5 min for both eyes. Schirmer’s test is a dry eye test where the moisture content in the eyes or production of tears is determined using special paper strips. Paper strip is inserted under the lower eyelid into the eyes. When the test result is normal- the filter paper shows more than 10 mm of moisture in 5 minutes. For Jyoti, this reading was less and was indicative of  mild dry eyes.  She was advised to discontinue contact lens use and was advised to use lubricating eye drops. She was asked to review after 2 weeks.

At 2 weeks follow-up her eye dryness was better. Given her borderline thin cornea, Dr. Jain recommended her photorefractive keratectomy (PRK) also called as advanced surface ablation (ASA). Kamothe man corrected his glass power without LASIK from AEHI

Vishal Agarwal* was a first-year law student who resided in Nerul. This young lad was also passionate about painting whose eyeglasses often used to be smeared with paint. This created a lot of hassle for him and he needed to frequently clean his glasses. One day, he finally decided to get rid of eyeglasses by undergoing LASIK surgery.

Based on one of his uncle’s recommendation, he booked an appointment with Advanced Eye Hospital and Institute (AEHI) in Sanpada, Navi Mumbai.

On the day of his visit, Vishal underwent pre-LASIK check-up. These tests revealed he had thin cornea along with compound myopic astigmatism with refractive error of -5.00/-2 x 180 in both the eyes.

Compound Myopic Astigmatism: In this type of astigmatism, light enters the eye from two focal points, both of which forms before the retina and at two different sites. The principal meridians are the meridians of greatest and least refracting powers. The amount of astigmatism is equal to the difference in refracting power of the two principal meridians.

LASIK eye treatment involves ablation of the cornea layers after raising a flap with microkeratome. Since, Vishal’s cornea was thin, LASIK was not the suitable vision correcting procedure for him. He was also not found to be suitable for surface ablation as his cornea was too thin and no form of ablative procedure was safe for him.

When he consulted Dr. Vandana Jain at AEHI, she advised him Toric Implantable Collamer Lens (ICL) implantation, which can correct refractive error without decreasing the corneal thickness or removing the natural lens. Toric ICL can also correct astigmatism. He chose to go for Toric IPCL which is the Indian phakic lens very similar to Toric ICL.

Gonioscopy test showed that the angle between iris and cornea was wide. Gonioscopy is performed prior to IPCL to rule out any form of angle closure.

Hence, he was referred to Dr. Rajesh Mishra, a Glaucoma specialist at AEHI who performed a YAG laser peripheral Iridotomy (PI).

YAG PI treatment involves creating a tiny hole on the iris, which aids in maintaining the smooth flow of aqueous humor from the anterior chamber to outside your eyes. It is done prior to IPCL so as to prevent pupillary block and future increase of intra-ocular eye pressure.

Vishal was then referred back to Dr. Jain who then surgically implanted IPCL in his right eye followed by left. After a month, Vishal showed stability in all the eye related parameters and was fully corrected of his refractive error.

LASIK or other excimer procedures are not the best treatment for getting rid of the spectacles when you have a thin cornea. Luckily, ICL/IPCL aka Phakic IOLs (Phakic intra ocular lenses) are specially designed lenses made up of biocompatible material – collagen. These lenses are surgically placed in front of the natural lens of the eye. They work just like contact lenses or glasses – by bending rays of light on to the back of the eye, the retina. The only difference is that instead of sitting on your eye like the usual contact lenses, these implantable contact lenses are placed inside your eye.

IPCL implantation has proven to preserve the best-corrected vision and low pre and post-operative complications. IPCL is a thin, soft and almost invisible once placed in your eyes. Additionally, it is not a good idea to assume that LASIK is ‘one size fits all’ procedure to remove glass power (refractive power). Having said this, IPCL implantation surgery also comes with its own criteria that can we can identify after a detailed eye check-up.

Navi Mumbai woman received best LASIK surgery after treating retinal hole at AEHI

Retinal hole refers to the eye condition of tiny breaks in the retina that is the innermost layer of the eye. Vashi youth regained confidence and normal vision after ICL surgery at AEHI

25-year-old Girish Kamat was working as a software developer in a MNC located at Airoli. After work, he used to learn dancing near his home at Vashi. Girish underwent pre-LASIK evaluation on the appointment day and consulted Dr. Vandana Jain. Refractive error showed that he had -2.50 power in his right eye while -2.25 in his left eye. While his eye pressure,  Orthoptic Work-up to check eye muscle balance; IOL Master to evaluate difference in length between the two eyes, if any, and Fundoscopy for retina evaluation were all normal.

However, as a part of pre-LASIK evaluation, Corneal topography test was also conducted to study the Cornea, which revealed that Girish’s cornea was mildly asymmetric. Further, Pachymetry showed that the thickness of his cornea was not enough. Abnormal corneal topography along with reduced corneal thickness made him unsuitable for laser vision correction procedures like lasik or femtolasik or smile lasik.

On one hand Girish was disappointed that he could not undergo Lasik but on other hand he was relieved that he got the right opinion. Dr. Jain then then explained him about Implantable Contact Lenses (ICL). Nerul woman freed from contact lenses by availing SMILE LASIK from AEHI

Nidhi Srinivasan* was a management employee trainee in a company located in Nerul, Navi Mumbai. After a long day at work, she started experiencing discomfort and pricking sensation in her right eye. Gradually, it turned red. She was worried that an eye infection she recently suffered hasn’t healed yet and is still showing its symptoms.

Contact lens users are at high risk of developing eye allergies and eye infections including keratitis (corneal ulcer). Such individuals are easy target for all kinds of micro-organisms like bacteria, virus, fungi etc especially if proper contact lens related hygiene is not maintained or there is misuse of contact lenses. Hence, it is common to see them catch conjunctivitis. These infections can give rise to eye pain, redness, swelling, light sensitivity, blurry vision, foreign body sensation, burning, itching etc depending on the nature and severity. Very rarely corneal infections can be sight threatening as well.

Dr. Jain then prescribed her few anti-allergic and lubricating eye drops and asked her to follow up after 3 weeks. She further advised her to discontinue wearing contact lenses completely.

Chembur Resident avails Implantable Contact Lens (ICL) for High Myopia at AEHI

Priyanka Wankhede had always been an active person all through her school and college days. She had interests in various fields like playing basketball, hosting several college events, etc., for which she always preferred to use contact lenses over eyeglasses. After completing her graduation, she started working as a HR professional in a company located in Vashi.

So, she was recommended Implantable contact lenses aka Implantable Collamer Lenses (ICL). These specialised advanced type of lens that are made from a copolymer of collagen i.e. Collamer. As collagen is the naturally occurring substance present in the human eye, it makes the lens extremely biocompatible. Additionally, with ICL surgery corneal curvature is not modified at all and this in turn reduces reflections and glare from bright lights which is sometimes an issue after Lasik done in high powers. Also ICL procedure is a reversible procedure and ICL can be easily removed from the eye is ever required. ICL is implanted through a very small incision inside the eye and hence its effect on the corneal sensations and the curvature is negligible. This reduces the risk of dryness predisposition after the surgery.

ICL are very thin and invisible once the ICL is implanted in the eye. It is safe, effective and improves unaided vision in all the cases. However, it does require proper planning and customization. All those who were not suitable for Lasik, Epi Lasik/PRK and Femto Lasik have ICL as a wonderful option to consider.
