Hyperopia or farsightedness is a common condition that affects approximately 25 per cent of the adult population. It refers to a vision problem in which the focusing power of the eye is too weak. It is a condition where either eye ball is shorter or corneal curvature is flatter than normal. So, the light entering the eye does not focus on the retina but behind the retina.

If the plus power is small then mostly distance vision is clear and glasses are required for near work only. However, as the power of corrective glasses becomes higher, most people need to wear glasses for both distance and near work. If people with higher plus power do not wear their glasses they not only get blurred vision but often complain of headaches and eye pain. Additionally, the glasses are thicker in the center and often people are uncomfortable with the look of the glasses on their faces. So, for many reason people with plus power want to get rid of their glasses and lead a glass free life.

Broad goal of Lasik in hyperopia correction is to increase the curvature of the cornea without inducing aberrations and ensuring the change is stable over time. There are many nuances to the correction of power with Lasik in these cases.

  1. Correct Power– Very often people who are using plus numbered glasses are wearing power equivalent to their manifest hyperopia. Because of accommodative capacity of the eye or the focusing ability most young people can partially correct their farsightedness by focusing or accommodating their own eyes. In order to uncover the true amount of farsightedness a cycloplegic refraction is performed by using special drops to temporarily paralyze the muscles that control accommodation. So as a first step we have to uncover the total hyperopia in most people by doing their refraction again after putting cycloplegic eye drops. So, what it means is that for most people is that the glass power that they are using and the actual power are different. Actual power is much higher than their glass power in most cases.
  2. Detailed Pre-Lasik evaluation– Even in cases who are considering Lasik for correction of plus power, the importance of detailed pre-Lasik evaluation cannot be over emphasised. Tests like corneal thickness, corneal topography, dry eye tests, eye muscle balance, retina and nerve checking are a must. In these cases, a special attention is paid to rule out any muscular imbalances. This detailed pre-Lasik evaluation helps us to identify those candidates where Lasik surgery should not be done as there is a greater risk of side effects and complications in future.
  3. Limit of correction– Lasik for plus power cannot be performed for very high powers. Usually anyone more than +5 to +6D are not good candidates for full correction. People with higher plus powers can get their numbers reduced but not fully corrected with Lasik. The reason for that is central corneal curvature cannot be increased beyond a certain limit with Lasik. Additionally, in these cases there will be high risk of regression (plus powers reappear) due to healing changes in paracentral cornea (epithelial hyperplasia).
  4. Newer Lasik Platforms for correction– It is important to ensure that the Lasik surgery is performed on the newer Lasik machine. With the older platforms, the chances of regression are much higher. Newer machines have better ablation profiles with large optical zones and a large transition zone. This helps in reducing the healing response at the paracentral cornea.

So, overall getting Lasik for correction of plus powers requires more thought and discussion with your Lasik surgeon.
